**Academic Year 2023 Physical Examination Guidelines For New Students**

1. All new students should submit the physical examination report in accordance with the School Health Act. Students directly promoted from undergraduate school to graduate school are also required to participate in this physical examination.

2. Physical Examination Hours: The physical examination is carried out in two separate sessions. Students in each school and department should participate at the designated hours (graduate students or executive program students can choose any session):

◆ 8 a.m. to 11 p.m., 4 September 2023 (School of Nursing, day-time division)

◆ 13 p.m. to 16p.m., 4 September 2023 (Department of Healthcare Management, Department of Information Management, Department of Exercise and Health Science, Department of Infant and Child care, Institute of Life and Death Education and Counseling.

3. Location: 2F, school main campus gym (disabled students should contact the health center by telephone before the examination day so that staff may be sent to provide assistance on the day of physical examination).

4. Please bring the physical examination fee of 600 NTD: Physical examination fee may be waived for students with proof of low-income household. Please bring a copy of the justification.

5. Please complete the basic information on the first page of the physical examination form and affix a photo in advance and bring the form to the examination location. Please do not send back the physical examination form in advance.

6. Guidelines before physical examination: Please do not eat anything for six hours prior to the physical examination. Please do not eat anything after 12 a.m. of the previous day. Any student with any chronic disease, such as hypertension or diabetics, should still take your medicine.

7. Guidelines on physical examination day:

(1) Wear simple clothing. Do not wear any metal object or bring any precious item.

(2) For female students, please inform the nursing staff if you are having your period on the examination day.

(3) For female students, please do not accept an X-ray if you are pregnant or may be pregnant.

(4)After completion of the physical examination, a breakfast will be provided.

8. If you cannot come to school on the physical examination day:

(1) If you have had a physical examination between June and September 2023(report of physical examination done before 1 June 2023 will not be accepted), please confirm whether the items under such physical examination were consistent with the items listed in the back of the school physical examination form. If any item is missing, please complete such item and transcribe the examination result onto the school physical examination form, with the official seal of the examination hospital affixed thereon and send back or deliver the form to the health center before 24 September 2023.

(2) You can carry out the examination in our cooperating hospital (Country Hospital) or your own medical institution between July 1st to  September 17th  2023. The items of examination must be the same as those listed in the school physical examination form. The results of the examination should be transcribed on to the school physical examination form, with the official seal of the examination hospital affixed thereon and send back or deliver the form to the health center before September 24th 2023.
